Hello! The only way to go to Azkaban is by the World Map, you can access this map by using Apparition to World skill (teleporting skill). But beware, dementors are really strong, if you go there being under level 7 and with basic equipment, and you get into a fight with 2 or 3 dementors, maybe you will be destroyed.
Mad Eye Moody is in this room that the Dumbledore's Army used to practise combat in Order of Phoenix, I won't say more so you can discover this place by yourself :P (but if you struggle too much, I can tell you). Tip: if you find the entrance to this room, try to enter various times.
